Did you realize that breast pump rental or purchase - even when prescribed as medically necessary for a premature infant - is not covered by most insurance policies, and is not even tax deductible as durable medical equipment?

Did you know that thousands of women each day have to express breast milk for their infants in dirty public restrooms? Would you prepare your lunch in a restroom?

Did you know that the American Academy of Pediatrics recommends breastfeeding for at least the first year of life (the World Health Organization recommends breastfeeding for at least the first two years of life), but many women stop after only six weeks, when their maternity leave is over, because they have little or no support to express milk at work?

Congresswoman Carolyn Maloney has introduced new breastfeeding legislation in the House of Representatives. Please write or e-mail your Congressional Representative, and express your support of this bill, which could make a HUGE difference to breastfeeding mothers. I am writing to urge you to support the Breastfeeding Promotion Act, HR 285.

As a breastfeeding mother of a 6.5 month old son, it would be a relief to know we have an advocate in Washington, D.C. Since returning to work, when my son was 6.5 weeks old, I have been pumping my breast milk 4 times per day at the office. With the help of a "hands free" bra, I can check my voice mail and e-mail, return phone calls, and do routine paperwork, while pumping - thus losing no productivity. I am extraordinarily lucky that I have a supportive employer that will allow me to remain dedicated to providing my son with the best, most natural food available. While my employer has accommodated my needs, unfortunately employers like this are the exception.

Thanks to my pump, my milk supply has not dwindled, and I am still able to nurse when I am with my son. I know many women that are unable to find the money to afford either the purchase or rental of a breast pump. I also know many women who have spent countless, frustrating, hours on the phone battling with Section 125 FSA representatives, to no avail, in hopes that they could purchase their pump, or pay for pump rental, through their Flexible Spending Accounts. There is apparently no consistency in the industry whether or not breast pump purchase or rental is covered by the pre-tax dollars set aside in this account. I know some women whose accounts have paid for their pumps under "durable medical equipment," while there are many whose accounts will not.

I would also like to ask you to consider another step in your support of breastfeeding women - insurance coverage for breast pump rental and purchase. Most insurance companies consider a breast pump as a "convenience" item. (Believe me, there is nothing convenient about pumping!) Breast pump rental or purchase is not covered by most plans - even for premature babies, or babies with special needs such as cleft palette or recessed chin, unless the baby is hospitalized. The American Academy of Pediatrics recommends breast milk for at least the first year of life (the World Health Organization recommends breastfeeding for at least the first two years of life), yet there is so little support for breastfeeding in our society that most women quit shortly after their maternity leave is over. In the long run, insurance coverage of pumps would undoubtedly save the insurance companies millions of dollars that they are currently spending on sick babies!

Please feel free to contact me if you need additional information about the needs of breastfeeding women in your district. I look forward to your efforts to support breastfeeding and pumping women throughout the nation.

Tips to Increase Supply

I thought we all could include some tips in ways to increase milk for those who think they are drying up or want more to pump.

I think all new moms need to remember that BF is all supply and demand. The first few weeks you may have a lot of milk or be engorged then after a month you may think your supply is dwindling, but in fact it is probably just getting into routine. So dont be alarmed if you only get 2 oz when you pump for 20 min. Your baby is probably only eating that much.

But if you think you need a boost try these tips:

Fenugreek is a herbal supplement you can purchase over the counter. It will not harm the baby. Take 2-3 pills for 2-3 days. You should notice a difference in supply by then. Your milk will smell like honey or syrup too!!

Nurse more often. Your body will think you baby needs more milk so it automatically thinks it should produce more. If you work, nurse more during the weekend and it should help for when you pump in the mornings.

I also find that it helps to stay well hydrated, and to pump consistently (if you are pumping) and for at least 10-15 minutes per session (double pumping). I have managed to maintain a great supply this way...lots for the baby and gobs in the freezer. (provided by Ridgedale)

I agree with being well hydrated, I try and drink at least 4 bottles of (500 ml) of water min a day. In addition, if you are pumping on a regular basis, I think your body gets used to it. If you do it regularly, your body will adapt. (provided by Wantubeprg)

Drinking to thirst, eating plenty of protein - eating a balanced diet in general, fenugreek, blessed thistle, oatmeal!, the commercial teas for lactating mothers... All of these things worked for me. (provided by Wawap)

There's also barley water - boil barley for a long time, strain it out and drink the water with honey. It's best warm. You can eat the barley with your oatmeal. Those whole grains will do you good. (provided by Wawap)

What as worked for me is rootbeer (because of the brewers yeast in it), the tea called mothers milk, and almonds(almonds beef up the fat in the milk)...(provided by Rfoxxy76)

I'm on Domperidone, taken for it's side effect which increases your prolactin level...increases milk production. Safe for baby and totally brought my supply back up, once it's up, slowly wean off medication and supply is still there. (provided by Jjbean)

There's an herbal tea called Mother's Milk that I've been drinking for 3 days now and it seems to be really helping I got it at The Vitamin Shoppe, but heard it's also available in health stores and then today I googled it and ordered it on-line. (provided by Ronniesaid)

My OB said that a glass of beer a day actually would help increase supply. He said that it isn't a widely known thing in the US because of stigma around women drinking and breastfeeding. He said simply drink a glass of beer 2 hours before a feeding (it will be out of your system). He laughed when I asked if a margarita would help... Nope just the beer (provided by DanaJyl)

The Let-Down Reflex (Milk Ejection Reflex)
What is the let-down reflex?

When you breast-feed, your baby's sucking stimulates nerves in your nipple. These nerves carry a message to your brain, and a hormone, called oxytocin, is released. Oxytocin flows through your bloodstream to your breasts, where it causes tiny muscle cells around your milk glands to squeeze milk out of the glands and into the milk ducts. This is known as the let-down reflex or the milk ejection reflex.

Once your let-down is working well (usually by 2 weeks after delivery), you may feel a pins-and-needles or tingling sensation in your breasts when you nurse or pump. Milk will usually drip from one breast while you are feeding on the other side. Sometimes your let-down will occur when you hear your baby cry or think about nursing your baby. A well-functioning let-down reflex helps ensure your breasts get emptied and your baby is easily getting milk.

Sometimes a woman's let-down reflex doesn't work as well as it should. This can cause breast-feeding problems. For example, you may have problems emptying milk from your breasts or your baby may not get enough milk.
What causes a poor let-down reflex?

Several situations may prevent the let-down reflex from working well.

* You may have severely sore nipples that cause you to tense up before each nursing.
* You may be stressed, anxious, or tense. For example, you may be trying to pump breast milk during a short break at work.
* You may be separated from your baby. For example, if you have to pump at home while your premature baby is still in the hospital.
* You may have had a previous breast surgery that has damaged the normal nerve pathways to the nipple, such as breast reduction or enlargement surgery. If you have altered sensation in your nipple after surgery (that is, your nipple is either somewhat numb or super-sensitive), it is possible that nerve damage from the procedure could interfere with your let-down reflex.

What can I do to improve my milk flow?

The following suggestions can help trigger the let-down reflex and improve milk flow:

* Try to nurse or pump in a place that is familiar, comfortable, and restful.
* Drink a beverage whenever you sit down to nurse or pump.
* Play soft music or do relaxation exercises before you nurse or pump.
* Gently massage your breasts before you nurse or pump.
* Have your partner give you a backrub before you nurse or pump.
* Put a warm washcloth or heating pad on your breasts, or take a warm shower before you nurse or pump.
* If you are pumping because you are separated from your baby, put a photograph of your baby by the pump.

Are there medical alternatives to improve my milk flow?

A synthetic (man-made) form of oxytocin was formerly marketed as a nasal spray known as Syntocinon. The drug was sometimes prescribed for mothers of premature infants who needed help conditioning their let-down reflex when using a breast pump. Syntocinon is no longer on the market. However, a compounding pharmacist can make the same drug with a prescription from your health care provider.

Breast Pumps

Hopingttc: I use the Medela PIS original - $250 from Babies R Us - I went back to work when Brayden was 2 1/2 months old (11 weeks old) and have been pumping 4 times a day with it - it's my best friend - never had any problems with it except that I don't think the motor is strong enough to yield enough milk when I double pump - so it takes me 25 minutes total to pump b/c I pump each side separately and get 5 oz per session....so I'm yielding about 20 oz a day and supplementing with formula for the remaining bottles.

Hopingttc: I used the Medela PIS original for almost 5 months until it broke - with 4 weeks left of pumping - now I'm using the Ameda purely yours - and I like it SO much better - I'm actually pumping 2-3 ozs more a session

Karah R: I have a PIS original-bought it brand new from www.kidalog.com. The price varies depending on US/Canadian exchange rate but I paid $142 shipped for it So I'd highly recommend that business in your comparison shopping.

Karah R: Not hospital grade-it's a Medela PIS. It's always worked very well for me, though! If I worked outside the home, I might have considered a hospital grade one if I were to need to nearly exclusively pump. I really only "needed" mine for pumping once or twice a day, and a few occasions when I needed to pump more than that, like prior to my surgeries last year, or conferences/continuing ed that would've been hard to take DS along for.

Ebay's another excellent place to look-there are both used and new, and you can just purchase new tubing and stuff if you wanted.

I have heard from mamas with the PIS Advanced that they like it but that it's not neccesarily worth the extra $$$$$$

The Ameda Purely Yours is another double electric pump that's good, and slightly cheaper. I know they sell it at the first site I mentioned.

Glynnis1214: I use the medela PIS also and I get 8 oz per session and that is only cuz i stop pummping when i get 4 oz per boob. I was originally gettin about 8 oz per boob which was giving me way to much milk for my fridge and freezer. I pump in bottles and no longer nurse on the boob which is the only reason I have made it this long!

Gingerjenny: i got the advent manual last time and threw it in the trash. it was a piece of junk. I didnt pump after that but I want to get a pump this time too.

Seastarz: I have the Ameda PY and I love it. Has never made me sore, and is easy to use.

Rejllj: I went back to work when Reed was 12 weeks and pumped twice a day at work until he was 9 months with the Medela PIS and never had any problems.

MayDay02: I have the manual Avent Isis pump. It worked fine for me, I only had to pump about 3 4oz bottles a week. If I wanted to pump any more than that though, I would probably go for an electric.

Southernbelle: I have the Playtex Embrace electric double breast pump. I really like it. I've only used it 2 times so far, though. The first time was when Mason was 1wk old and I was able to get 4oz in a 30min sitting, so I think it did well. The only times I have pumped were b/c I wasn't comfortable enough to nurse him while out...we were having latch issues then. Now I know it will be nice to have so that I can go the salon or get a pedicure or something; a little mommy time!

MistyDC28: I have a Bailey Nurture III. This is what I used last time too. I hadn't ever used a hospital grade pump until Wesley was in the hospital 3 weeks ago and man, what a difference! It was great, but the Nurture works well too.

KeriHallman: Medela pump in style has been wonderful!!!!! I could not imagine pumping for as long as I have with another pump! I had a first years pump from Walmart with my first and it was crap! I would pump all day long to get one 8 oz bottle! I can pump almost 35 oz a day with the medela.

Lara31: I rented an Ameda lactaline personal and I like it but I've heard the medela ones are the best.

MsMoon880: I pumped every day for almost a year w/ the Medela Pump in Style. It's great!

When I first had Delainey I borrowed a friends PIS and was unable to get it to work for me when Delainey self weaned at 9 months. When I found out I was expecting twins it became my mission to pump if I couldn't nurse so I bought a PIS and was given a Ameda from my local lacation consultant ( they wanted me to write a paper on the pump so that they could get more $$ for grants and so they gave me the pump)...

I like to Ameda when I am going somewhere because it is smaller but the Medela is my fav :-)

2cutekitties: I like my Avent Isis Cheap and works well but if you need to pump daily I would buy a different one....

ainflower: We started with a double electric hospital grade Lactina Medela Pump that the insurance rented us for 3 months while babies were in the hospital. The LC told us if we found a used one online, not to buy it cause most likely it would be a stolen item (someone making money on an un-returned rental) cause they are like $500 or something.

My mom's friend gave me an old Medela Pump InStyle, but the LC told me these are only designed for a single user cause they could not be sterilized. So I bought my own Medela Pump in Style (2005 Model) from momscorner.com for $189. Its very convenient as it has a battery pack, so when you can't find an outlet, you can still pump. I take it to work a few times a week and use it in the hospital when my DS's have operations and such. I also use it when I get a blocked duct to try empty my BBs fully. I had to get a bigger horn size as the horns they came with were too small. If you can meet with a lactation consultant, they can tell what size will work best for your BBs!

My friend loves the Avent Isis manual pump (she has very fast letdown and fast flow BBs! She was able to pump exclusively with that for 9 months as her DS couldn't handle nursing.) I haven't tried it, but in retrospect, it seems like it might be a little easier to handle than the double electric at times.

Truthfully, I wouldn't say I love any pump, cause I really do not like to pump, but I am grateful it has helped me to be able to breastfeed my preemie twins!

Tararaboomdeea: when noah was in the nicu i used the medela symphony. i loved it! i rented it, so i have no clue what it would cost new. now i use an avent isis and like that too. i have only pumped a couple of times since he's been home when i have felt too full. he's my breast pump

Nursing Bras

MayDay02: I always wore (and still do, at night anyway) "cami" style stretchy bras. They are the same "shape" and look kind of like sports bras but they aren't as tight and umcomfy as sports bras. I love them!! I have a few "Arizona" brand from Kohls that I love. They are cheap, too. And, I beleive they go from an XS to a XXL. Perfect for big engorged BBs!

Kimandarts peanut: I got mine through www.figleaves.com (free shipping!). It was an Elle MacPherson nursing bra and I loved it. I used the Bravado ones in the early months, but they were sooo not sexy. The EM one was lavender, silky and great! Quite an improvement.

They are very sofy and have underwire.I wear a 40E and they fit so good and comfy

Breast Creams

Karah R: Lansinoh, or any other pure lanolin cream will help you out--I think there's another one called PureLan that is as just lanolin, so you don't have to wash it off or anything like that.

Hoppingttc: my doula always told me that the best remedy is to express some milk on your nipples - so I never used any of the creams

Miraclesonx1: A LC suggested something called Soothies. They are these gel things you put in the ridge and once cooled, place them on your nipples. It feels SO good!

EARTH MOMMA ANGEL BABY NATURAL NIPPLE BUTTER

All natural stuff, NO LANOLIN. (My experience with that stuff made matters worse!)

I got mine in a gift package but found it at a natural food store.
They made all sorts of natural good stuff. Even butt balm for baby. NICE!

Nancymom: I agree with rubbing breastmilk on your nipples and also Vitamin E drops are nice too. It is not as sticky as the lanolin.

Nursing Pads

Gimmenoting: i've been using Lilypadz since day one and i absolutely love them! they're really soft thin silicone and stick right on...after you use them for a while, you'll find that they start losing their stickiness a little bit...when this starts to happen, i brush mine nice and stern with a toothbrush and soap, and/or boil them for about five minutes...it helps to bring them back...i can't imaging living without them!

southernBelleMommy: Right now I am using Johnson's nursing pads because they are soft.

CelticRose73: I have used them all, and only used Lansinoh. They are more expensive, but you don't have to change them as often.

Karah R: I always used Lansinoh for the same reason Kelly mentioned, and found the Johnson and Johnson ones, though cheaper, filled up and leaked more easily and had to be changed more often. I always felt dry with the Lansinoh ones, even if they were totally full with milk.

I DO plan on buying the LilyPadz, though, because I have been hearing raves on them for a long time, and like the idea of something reusable that actually stops leaking when it starts.

2cutekitties: I use Avent, lansinoh ones irritated my skin because I sweat too much, they don't breathe as well because of the plastic backing.

Fooey808: I have been using the lansinoh ones and they are great. I also like the first years (from babies r us) ones too, they have lanolin already on them. I think I want to try those lilypadz.

MsMoon880: I tried several, because I leaked a ton, and I liked the Lansinoh and Medela brands equally well. They both have that gel stuff in them like diapers have, so they are thinner than most pads, but way more absorbant. Once I stopped leaking regularly, I wore cloth pads just in case.

Nellrush: I have always used the curity...never have leaked and really don't even notice they are there.

Bottles

Laineys Momma: so far we have found that the playtex ones work as well as the Avent....

Fooey808: The only bottle Megan will take is the dr browns. She only gets the occasional bottle and it hasn't been easy.

Tararaboomdeea: when i weaned jack, we used the nuk nipples and they worked great. they're noy shaped like your nipple, but baby has to work to get some, so it's like bfing. we used them for emily too and both did well with them. everything else was way too fast. good luck!

Scamp: We've used Playtex since the first week home from the hospital. She will take the silicone nipples but no others! I'm sure because she is so used to the silicone nipple shield I'm using...

Ainflower: Our boys started with the NICU bottles, which are 2 oz bottles with normal looking rubber (not latex) nipples. Some had one hole in them and some had 3. Then we tried playtex with the dropins. They liked those alright, but it seemed they still sucked a lot of air. Then we tried the Dr. Brown's both the wide ones and the narrow ones, which is normallly what they use now. They didn't like the Avent or the Evenflow for some reason...

Casharina: We used a syringe and now a sippy cup, the Nuk brand, like tararaboomdeea said, they have to work at it so it's more like bf and it has worked great for us. She too only gets it every now and then. If you start with a sippy cup, then you won't have to wean from a bottle to one. Some hospitals only use cups to feed babies, even premature babies. (I looked it up when trying to decide what to use! LOL I'm a bf freak, I wanna do everything the right way!!)

Egills: When I was bf the only bottle Hope would take was Dr. Browns. She still is what I call a bottle snob and will only use Dr. Browns.

I love them. They aren't as difficult to clean as you might think. All the parts come undone and I just throw them in the dishwasher.

Karah R: When DH fed Carter bottles of EBM, we used Avent bottles and they worked well for us.

Seastarz: We use the Avent one too. Works great for us!

Miraclesonx1: Just the medella orthodontic ones but I rarely bottle feed. Maybe once per month.

Blcalt: All 3 of mine love Dr. Brown's. It worked great with Alexis's reflux. Nicholas get one bottle a day. I pump at work.

2cutekitties: We have used Avent once or twice but he definitely prefers me .

rejllj: Reed would only take me or the playtex nursers.
